Independent Provider Agreement

The final signature copy must identify the PNW Network Operator legal entity and complete Schedule 1, including notice details, payout timing, governing law, venue, payment structure and the approved market addendum.
The standard completed-job split is an 80% provider payout and a 20% PNW network fee, unless different written terms are shown before acceptance.
Providers review available job information and may accept or decline. PNW does not guarantee work, income, territory or margin.
Signing the agreement or displaying a badge does not create hauling, disposal, construction or local operating authority.
